• Εμβολιασμός κατά της νόσου COVID-19 & Θεραπεία της οστεοπόρωσης

    Η οστεοπόρωση, ως χρόνια πάθηση, απαιτεί διαρκή ιατρική παρακολούθηση για τον περιορισμό του κινδύνου πρόσθετης οστικής απώλειας, και την εμφάνιση κατάγματος. Η καθυστέρηση ή η απότομη διακοπή ορισμένων αντιοστεοπορωτικών φαρμάκων μπορούν να έχουν σημαντικές επιπτώσεις στην υγεία των ασθενών.

    Η πανδημία COVID-19 έχει επηρεάσει σοβαρά όλες τις πτυχές της ζωής παγκοσμίως και επηρέασε επίσης ασθενείς με μυοσκελετικές παθήσεις καθώς και τη φροντίδα που τους δόθηκε. Η δυνατότητα εμβολιασμού εγείρει ερωτήσεις για το αν υπάρχει αντένδειξη με τη θεραπεία της οστεοπόρωσης.

    Τα εμβόλια για τον COVID-19 (δηλαδή έναντι του νέου κορονοϊού SARS-CoV-2) που έχουν λάβει και θα λάβουν σχετική έγκριση μπορούν να χρησιμοποιηθούν με ασφάλεια σε ασθενείς με οστεοπόρωση, καθώς και σε ασθενείς που λαμβάνουν φαρμακευτική αγωγή.

    IV διφωσφονικά

    Συστήνεται διάστημα 1 εβδομάδας μεταξύ της ενδοφλέβιας έγχυσης του IV διφωσφονικού και του εμβολίου έναντι στη νόσο COVID-19 ώστε σε πιθανή εμφάνιση αντίδρασης οξείας φάσης να είναι εφικτή η διάκριση εάν προέρχεται από το ενδοφέβλιο διφωσφωνικό ή από το εμβόλιο κατά της COVID-19.

    Denosumab 

    Συστήνεται διάστημα 4-7 ημερών μεταξύ της ένεσης Denosumab και του εμβολίου έναντι στη νόσο COVID-19 ώστε να είναι εφικτή η διάκριση πιθανής αντίδρασης στο σημείο της ένεσης από που προέρχεται εάν γίνει στον ίδιο βραχίονα με το εμβόλιο. Εναλλακτικά, η ένεση Denosumab μπορεί να γίνει στον άλλο βραχίονα (στο άνω τμήμα του) ή σε διαφορετικό σημείο χορήγησης (κοιλιακή χώρα ή  επάνω μέρος των μηρών) σε περίπτωση που χρειάζεται να γίνει ταυτόχρονα με το εμβόλιο κατά της COVID-19. Η θεραπεία με Denosumab δεν θα πρέπει σε καμία περίπτωση να καθυστερεί πάνω από 7 μήνες από την προηγούμενη δόση Denosumab.

    Λοιπές αγωγές (από του στόματος διφωσφονικά, τεριπαρατίδη, ραλοξιφένη, ασβέστιο, βιταμίνη D) δεν χρειάζονται κάποια αλλαγή.

  • Επικαιροποίηση των Κατευθυντήριων Οδηγιών του ΕΛΙΟΣ για την αναβολική θεραπεία

    ΕΠΙΚΑΙΡΟΠΟΙΗΣΗ ΤΩΝ ΚΑΤΕΥΘΥΝΤΗΡΙΩΝ ΟΔΗΓΙΩΝ ΤΟΥ ΕΛΙΟΣ ΓΙΑ ΤΗΝ ΑΝΑΒΟΛΙΚΗ ΘΕΡΑΠΕΙΑ

     

    Καθώς μετά την εκπόνηση των κατευθυντήριων οδηγιών του ΕΛ.Ι.ΟΣ για την αντιμετώπιση της οστεοπόρωσης, διάφορες ρυθμιστικές αρχές σε όλο τον κόσμο (ΗΠΑ-Καναδά, Ευρώπη, Ιαπωνία) ενέκριναν τη Ρομοσοζουμάμπη (Romosozumab), μια νέα αναβολική αγωγή με διαφορετικό μηχανισμό δράσης από την τεριπαρατίδη και την αμπαλοπαρατίδη για την αντιμετώπιση της οστεοπόρωσης, θεωρήσαμε απαραίτητο να επικαιροποιήσουμε τις υπάρχουσες οδηγίες στο κεφάλαιο της αναβολικής αγωγής.

     

    ΡΟΜΟΣΟΖΟΥΜΑΜΠΗ

    Η ρομοσοζουμάμπη είναι ένα αναβολικό φάρμακο που αυξάνει την οστική παραγωγή και μειώνει την οστική απορρόφηση. Χορηγείται μια φορά τον μήνα (210 mg) για 1 έτος και η χορήγησή του συνοδεύεται από μεγάλες αυξήσεις της οστικής πυκνότητας τόσο της σπονδυλικής στήλης όσο και του ισχίου συνέπεια της διέγερσης της οστικής κατασκευής [1].

    Δυο μεγάλες κλινικές μελέτες διενεργήθηκαν για να ελέγξουν την αποτελεσματικότητα του φαρμάκου στη μείωση των σπονδυλικών και μη σπονδυλικών καταγμάτων. Η μελέτη FRAME (Fracture Study in Postmenopausal Women with Osteoporosis) [2] περιέλαβε 7180 μετεμμηνοπαυσιακές γυναίκες στις οποίες χορηγήθηκε μηνιαία ρομοσοζουμάμπη ή placebo. Η ανάλυση των δεδομένων έδειξε ότι η ρομοσοζουμάμπη μείωσε σε 1 έτος τα σπονδυλικά κατάγματα κατά 73% σε σύγκριση με την ομάδα placebo [Σχετικός κίνδυνος (ΣΚ) 0.27; 95% διάστημα εμπιστοσύνης (ΔΕ) 0.16-0.47], αλλά όχι τα μη σπονδυλικά κατάγματα. Τους επόμενους 12 μήνες όλοι οι ασθενείς συνέχισαν με δενοσουμάμπη. Στους 24 μήνες οι ασθενείς που είχαν ξεκινήσει με ρομοσοζουμάμπη και συνέχισαν με δενοσουμάμπη είχαν 75% μείωση των νέων σπονδυλικών καταγμάτων (ΣΚ 0.25; 95% ΔΕ 0.16-0.40). Τέλος, στη μελέτη επέκτασης της FRAME στην οποία χορηγήθηκε δενοσουμάμπη 1 ακόμα έτος (συνολικά 1 έτος ρομοσοζουμάμπη και 2 έτη δενοσουμάμπη) διατηρήθηκε όχι μόνο ο μειωμένος κίνδυνος κατάγματος αλλά και οι αυξήσεις της οστικής πυκνότητας στο ισχίο και τη σπονδυλική στήλη [3].

    Στην ARCH (Active-Controlled Fracture Study in Postmenopausal Women with Osteoporosis at High Risk) μελέτη [4], 1 έτος θεραπεία με ρομοσοζουμάμπη ακολουθούμενη από 1 έτος αλενδρονάτη συγκρίθηκε με τη χορήγηση 2 ετών αλενδρονάτης σε μετεμμηνοπαυσιακές γυναίκες με υψηλό κίνδυνο για κάταγμα. Η μελέτη έδειξε ότι η αλληλουχία ρομοσοζουμάμπη/αλενδρονάτη συγκρινόμενη με την αλενδρονάτη/αλενδρονάτη συνοδεύεται από 48% μείωση των σπονδυλικών καταγμάτων σε 24 μήνες (ΣΚ 0.52; 95% ΔΕ 0.40-0.66), 38% μείωση του κινδύνου κατάγματος του ισχίου [Λόγος κινδύνου (ΛΚ) 0.62; 95% ΔΕ 0.42-0.92] και 19% μείωση του κινδύνου μη σπονδυλικού κατάγματος (ΛΚ 0.81; 95% ΔΕ 0.66-0.99).

    Η ρομοσοζουμάμπη έχει επίσης χορηγηθεί και σε άνδρες με οστεοπόρωση [5], χωρίς προς το παρόν να έχει πάρει έγκριση.

    Τέλος, μια συστηματική ανασκόπηση σχετικά με τις προτιμήσεις των ασθενών ανέφερε ότι οι ενέσεις δεν ενοχλούν τους ασθενείς εάν αυτές δεν είναι συχνές [6].

     

    ΑΝΕΠΙΘΥΜΗΤΑ ΣΥΜΒΑΝΤΑ

    Στη μελέτη FRAME [2] δεν υπήρχαν διαφορές μεταξύ της ομάδας της ρομοσοζουμάμπης και της ομάδας placebo όσον αφορά τα καρδιαγγειακά συμβάντα ενώ στη μελέτη ARCH [4] παρατηρήθηκε ανισορροπία το πρώτο έτος θεραπείας με 50 ασθενείς της ομάδας της ρομοσοζουμάμπης σε σύγκριση με 38 της ομάδας της αλενδρονάτης να αναφέρουν μείζονα καρδιαγγειακά συμβάντα [Odds ratio (OR) 1.31; 95% ΔΕ 0,85-2.00]. Επειδή στη μελέτη ARCH η σύγκριση έγινε με την αλενδρονάτη συζητείται η υπόθεση η αλενδρονάτη να μειώνει τα καρδιαγγειακά συμβάντα [7]. Το θέμα όμως αυτό είναι αβέβαιο την παρούσα στιγμή και παρά το γεγονός ότι οι αριθμοί των καρδιαγγειακών συμβαμάτων είναι μικροί και τα διαστήματα εμπιστοσύνης μεγάλα, ο Ευρωπαϊκός Οργανισμός φαρμάκων [8] (όπως και οι υπόλοιποι ρυθμιστικοί οργανισμοί [9]) έθεσε μια προειδοποίηση στην έγκριση του φαρμάκου ώστε να μην χορηγείται σε ασθενείς με ιστορικό εμφράγματος του μυοκαρδίου ή αγγειακού εγκεφαλικού επεισοδίου.

    Επίσης στη μελέτη FRAME [2], αναφέρθηκε μια περίπτωση οστεονέκρωσης της γνάθου στην ομάδα της ρομοσοζουμάμπης και καμία στην ομάδα placebo. Μια ακόμη περίπτωση παρατηρήθηκε κατά τη διάρκεια της χορήγησης δενοσουμάμπης σε μια γυναίκα που είχε πάρει πριν ρομοσοζουμάμπη. Στη μελέτη ARCH [4] περιπτώσεις οστεονέκρωσης της γνάθου δεν αναφέρθηκαν κατά το πρώτο έτος θεραπείας ενώ 2 περιπτώσεις αναφέρθηκαν κατά το δεύτερο έτος, 1 στην ομάδα που πήρε μόνο αλενδρονάτη και 1 στην ομάδα που πήρε ρομοσοζουμάμπη και συνέχισε με αλενδρονάτη.

    Όσον αφορά τα άτυπα κατάγματα του μηριαίου στη μελέτη FRAME [2] παρατηρήθηκε 1 τέτοιο κάταγμα σε ασθενή που έλαβε ρομοσοζουμάμπη.

    Κατά τη διάρκεια της μελέτης ARCH [4] αναφέρθηκαν 6 άτυπα κατάγματα του μηριαίου κατά το δεύτερο έτος της μελέτης, 4 στην ομάδα που έλαβε μόνο αλενδρονάτη και 2 στην ομάδα ρομοσοζουμάμπη/αλενδρονάτη.

    Τοπικές αντιδράσεις στις θέσεις της ένεσης παρατηρήθηκαν σε ποσοστό 3% στην ομάδα placebo και 5% στην ομάδα της ρομοσοζουμάμπης κατά τη διάρκεια της μελέτης FRAME. Παρόμοια ποσοστά παρατηρήθηκαν και στη μελέτη ARCH [9].

    Τέλος, η θεραπεία με ρομοσοζουμάμπη δεν συνοδεύεται από ανάπτυξη νέων όγκων κατά τη διάρκεια των μελετών που χορηγήθηκε.

     

    ΣΥΜΠΕΡΑΣΜΑΤΑ

    Η ρομοσοζουμάμπη μειώνει τα σπονδυλικά, μη σπονδυλικά και τα κατάγματα του ισχίου σε μετεμμηνοπαυσιακές γυναίκες με οστεοπόρωση και αυξημένο κίνδυνο κατάγματος.

     

    ΣΥΣΤΑΣΕΙΣ

    • Η ρομοσοζουμάμπη αποτελεί φάρμακο πρώτης επιλογής για μετεμμηνοπαυσιακές γυναίκες με οστεοπόρωση, και αυξημένο κίνδυνο για κάταγμα (Bαθμός Α).
    • Η συνιστώμενη δόση είναι 210 mg με υποδόρια ένεση μηνιαία και η διάρκεια θεραπείας είναι 1 έτος.
    • Δεν συνιστάται η χορήγησή της σε ασθενείς με ιστορικό εμφράγματος του μυοκαρδίου ή αγγειακού εγκεφαλικού επεισοδίου.
    • Οι μετεμμηνοπαυσιακές γυναίκες που συμπληρώνουν την αγωγή με ρομοσοζουμάμπη συστήνεται να συνεχίσουν με αντικακαταβολική αγωγή ώστε να διατηρήσουν τα κέρδη της αύξησης της οστικής πυκνότητας και του μειωμένου κινδύνου κατάγματος (Βαθμός Α).

On the contrary, the main goal of this website is to assist the efforts of doctor and familiar scientists by providing information, audiovisual material (pictures, videos, patterns, etc) and access to the association’s services. The subsidiary goal of this website is to provide information to the public concerning these diseases in an effort to increase its awareness on their existence and seriousness.

3.Association’s responsibility. 11.Applicable law and final provisions. If any unlawful and/or unenforceable term would be lawful or enforceable if part of it were deleted, that part will be deemed to be deleted, and the rest of the terms will continue in effect.
